    Dear Parent/Guardian

Your child ha s been in contact with a person who has tested positive for COVID-19 or is presumed to have COVID-19 while attendin g __________ school. The Snohomish Health District Health Officer mandates you quarantine your child (i.e., stay home and away from public places with no visitors, not attend any other group child care setting) for 14 days after the last day they were exposed to the person with COVID-19. For your child, this means staying home until ___________ , unless there is further exposure from someone else who has COVID-19. You will find more information about what to do if you are a contact of a COVID-19 case on the attached handout .   Please call the health department at 425-252-5415 to update your profile and to ask any questions or request for additional information and assistance.

Anybody living in the same household as someone with COVID-19 and is not ill with COVID-19 themselves should be in quarantine both:

    · Throughout the home isolation period of the person with COVID-19 and

    · 14 days after the home isolation period of the person with COVID-19 ends

If you, your child or any of your household members are currently experiencing any symptoms consistent with COVID-19   (e.g., fever, cough, sore throat, runny nose, etc.), we encourage you to get tested. Please contact the Snohomish Health District at 425-252-5415 and ask to talk with a public health nurse to discuss the symptoms and next steps. To assist the health department in providing you with timely recommendations, please provide the name of the school your child attended when you call.   If your child remains symptom-free through the duration of your quarantine period, we recommend they get tested a few days before their quarantine ends.

Because we are in a pandemic, we need to follow up with you for the duration of your child’s quarantine period. You will be contacted daily via text on the number you provide to which we ask you to do your best to respond promptly. If you prefer to receive an automated phone call or email message, please call the number above to notify us so we can make the appropriate changes.

    Thank you,

    Snohomish Health District
