    October, November and December of 2014 to be exact … 


    Did you receive any medical treatment?

    Did you have a procedure done over winter break? Or see that specialist? 

    This won ’t be for the new glasses you got, or that tooth you had fixed, but serious medical treatment. 

    If you did, and charges applied to your medical deductible (the portion you pay), please let us know by April 30, 2015. 

    If you aren ’t certain, check your Explanation of Benefits forms (EOBs) or call Premera at 1-800-932-9221 to inquire about any charges that might have applied to your deductible in October, November and December 2014. 

      





    Why, you ask?? 





    th Quarter Deductible Carry Over 


    The charges you paid for out of pocket may be credited toward your current year ’s deductible. Applying your 4 th quarter deductible to your 2015 deductible may help you reach your calendar year deductible quicker, resulting in earlier payment of benefits at the plan rate. 

    The benefits department will share this information with UHC in hopes of potentially crediting your deductible for the charges incurred during the previous 4 th quarter.

    Attach any supporting documents showing this information.   This includes a billing statement showing a line item for deductible or an EOB showing no insurance payments made due to deductible. 

    UnitedHealthcare (UHC) is allowing Everett Public Schools a one-time-only submission to credit any applicable 4 th quarter carryover to employees ’ 2015 benefit year deductible. UHC will only accept this as one document, submitted once by our benefits department. Therefore, individuals will be unable to report this information directly to UHC or have their accounts credited after April 30, 2015. The Everett Public Schools benefits staff, in strict compliance with HIPAA regulations, will compile and securely submit the 4 th quarter deductible amounts for all Everett Public Schools employees to UHC. 





    How will you know if your information has been updated?


    If any credits can be applied, previously submitted claims may be re-processed to account for the credit. If re-processing occurs and credits can be applied, you will be able to review revised UHC EOBs noting the adjustments.
